Sampling: Download a Custody Form here. Select samples that are
representative of your product both in nature of the product
and in the packaging. Where applicable, adhere to FDA and USDA
sampling guidelines. Please provide the following information
for all samples submitted: Company Name, Address, Phone and
Fax and a contact person, Purchase order number,
complete and unique sample description and lot number for each
sample sent (This will be on your test certificate.) and a
detailed list of tests required and estimated analyze level if
known.
Confidential: Results will NOT
be released to others without your written consent. Nor will
results be faxed or phoned to another location without
written consent. If you need results sent to anyone
other than yourself or several different locations, simply put
your request in writing (email is acceptable).
Samples and Containers: Samples are
retained for 14 days from the time they arrive at the
laboratory. If the sample is perishable it will be discarded
immediately after the analysis is completed. Please provide
samples in air/leak-proof containers. Sample pick-up can be
arranged by request. Please call our office for pick-up times
and current pricing. CHEMICAL TESTING: Ship in a protective
container. If a sample is to be tested for both Micro and
Chemistry, please send two samples if possible. This will
speed up the turnaround time and eliminate possible
cross-contamination.
Results: An official Test
Certificate will be e-mailed to our clients. Upon
request, results can also be phoned. You may call anytime for
the status of your sample.
Reporting Procedures: All
information is presented in good faith. As a mutual protection
to clients, the public and ourselves, all reports are
submitted as the confidential property of our clients and
authorization for publication of statements, conclusions or
extracts regarding our reports is reserved pending our written
approval.
Turnaround Time: Microbiology tests
are performed within 48 hours of receipt and results are
reported as soon as they are completed. Some Micro tests take
as long as 7 days. Wet Chemistry, Minerals or Organic
Chemistry are normally reported within 10-12 working days.
Call concerning your turnaround time.
Rush Testing: Rush analyses are
available on a limited number of tests. If you need rush
analyses, please call in advance for details and specify
"rush" on your work order. An extra fee may apply to rush
analyses.
Repeat Analyses: We repeat analyses
at the client's request and invoice for the repeated analyses
ONLY if they confirm the initial results. If the result is
different, a Corrective Action form will be filled out and
supplied to our client.
Testing Methods: Unless otherwise
notified, all analyses are performed as per AOAC, USP, FDA,
USDA, EPA or other such testing procedures as deemed
applicable for your sample. Custom methods are always welcome.
Please note any custom methods will be run along with an
approved method.
Quality Control: Advanced Laboratories®, Inc. meets and exceeds the Quality Control Standards
set forth by USDA-FSIS, AOAC, FDA and the State of Utah.
Subcontracting: Either in-house or through our strategic partner laboratories, Advanced Laboratories® can provide all of your analytical analysis needs, with one-stop sample shipping.
Testing Charges: All accounts are
set up as Net 30 days. For first time clients, please refer to the New Account Setup page. All accounts greater than 60 days late will be
charged a $50 service fee and will acquire an additional $5
fee every 10 days until the account is paid in full, including
all service fees. Any account greater than 90 days late will
be charged a $100 service fee and will acquire an additional
$10 fee every 10 days thereafter until the account is paid in
full, including any and all service fees. Any cost acquired for legal fees will be added to
the account. Please help keep the cost of your testing down by
paying invoices in a timely fashion.
